This nationwide National Cancer Database analysis evaluated whether adjuvant radiotherapy improves survival in patients with margin-negative, node-positive pancreatic ductal adenocarcinoma after pancreaticoduodenectomy. Using propensity-matched cohorts, adjuvant radiotherapy provided only a modest survival benefit overall and for stage IIB patients, with no benefit observed in stage III patients. Findings suggest that routine use of adjuvant radiotherapy in R0N+ pancreatic ductal adenocarcinoma should be individualized, balancing limited benefit against treatment burden (https://doi.org/10.1245/s10434-026-19566-5).
