Predictors of Early Adverse Outcomes After Heart Transplantation

Abstract

Purpose: In-hospital mortality and prolonged hospitalization serve as key indicators of complications following heart transplantation. However, national data identifying contemporary predictors of these outcomes remain limited. This study aims to determine donor and recipient factors independently associated with in-hospital mortality and prolonged hospital stay (>30 days) following heart transplantation. Methods: The UNOS database was queried for adult isolated heart transplant recipients (1/2000-6/2025). Primary outcomes were in-hospital mortality and hospitalization >30 days. Variables significant on univariable analysis (p<0.05) were refined by LASSO and entered into separate multivariable logistic models. Adjusted odds ratios (AOR) with 95% CI were reported. Results: Among 59,866 adult heart transplant recipients, approximately 39,076 (45.4%) experienced prolonged hospitalization (>30 days) and 5,687 (9.5%) died during the index hospitalization. Variables independently associated with both prolonged hospitalization and in-hospital mortality (respectively) included: higher recipient, prior cardiac surgery, cerebrovascular/stroke donor cause of death, head trauma donor cause of death, ventilatory support at transplant, and serum creatinine at transplant, among others (Tables 1 and 2). Conclusion: In this national analysis, markers of severe preoperative illness and higher-risk donor characteristics were associated with in-hospital mortality and prolonged stay. Recognition of these predictors may improve perioperative management and risk stratification in heart transplantation.
