Chronic pain adversely affects the quality of life and imposes a large burden on health care systems. Though much effort has been directed at elucidating the role of genetic risk factors underlying pain, susceptibility, and treatment responses, the pain field can benefit tremendously from entirely new approaches rendering novel perspectives on how we treat pain. This chapter highlights advances in the field of pain epigenetics including DNA methylation, posttranslational modifications of histones, and RNA-mediated gene silencing. Investigating how epigenetic alterations can lead to chronic pain, influence risk factors, and help determine treatment response can lead to the identification of novel pain targets or intervention strategies to complement existing therapies. Patient-specific diagnosis and treatment strategies based on the unique profile of the individual will be highly beneficial in treating chronic pain.
