Chapter 21 - Innovations for aging in place

Abstract

Aging in place community home-based care
We examine factors which support or serve as barriers to aging in place, theoretical models that frame relocation decision-making, and innovative aging-in-place service models. Community-based models such as the Village concept, Naturally Occurring Retirement Communities, Cohousing, and Fureai Kippu or Time-banking are promising approaches. Professionally facilitated programs such as Community Aging in Place: Advancing Better Living for Elders (CAPABLE), Program of All-Inclusive Care for the Elderly (PACE), Support and Services at Home (SASH), and Tiger Place demonstrate effectiveness in helping participants age at home. Promising approaches to support people with dementia and their care partners include Care of People with dementia in their Environments (COPE), Adult Day Service Plus Program, Dementia Family Care Programme (DFCP), a family counseling and support intervention, and Tailored Activity Program. We also examine technologies that support aging in place, as well as intergenerational innovations, and emerging age-friendly criteria for community planning.
