Abstract
Racial/ethnic disparities in pain care affecting non-Hispanic Blacks, Hispanics, and other minority, primarily disadvantaged populations have been a pervasive challenge to pain care providers for decades. While some improvements toward the delivery of equitable, high quality pain care to all have been achieved, much more is required. In mid-2020, the COVID-19 pandemic and the resurgence of the Black Lives Matter movement have motivated many healthcare providers from diverse specialty areas to advocate for greater recognition of the influence of systemic racism on the organization and delivery of healthcare services and for medical and healthcare practitioners to commit to establishing healthcare systems that are free of racist policies and practices. Within this larger social consciousness, the purposes of this chapter are 1) to provide a summary of current evidence pertinent to pain care disparities and the factors that foster such disparities; 2) to identify critical provider attitudes and behaviors that are antithetical to the delivery of equitable pain care and for which the pain care provider is responsible to address, and 3) to clarify the changes that are required of the provider and to specify actions that can be taken by pain care practitioners to succeed in making the necessary changes. It is hoped that the chapter will encourage pain care practitioners to undertake the challenging but achievable steps needed to eliminate disparate pain care to all racial/ethnic and other marginalized groups.
