Inflammatory bowel disease (IBD), including both Crohn's disease and ulcerative colitis, is a chronic remitting and relapsing disease. Recent medical and surgical advancements have transformed the way we manage the disease. Endoscopy, specifically colonoscopy, plays a vital role in the management of the disease. From the initial diagnosis to disease surveillance and with recent therapeutic interventions, colonoscopy plays a significant role, along with medical and surgical treatments, in the management of CD. In this chapter, we discuss how colonoscopy can aid in the initial diagnosis of the disease. We also discuss various types of scoring systems used to objectively assess the severity of the disease and how these scoring systems can help us monitor the disease. We also discuss recent advances in technology and the development of newer scoring systems, as the advent of artificial intelligence will help shape the future of this field. Finally, we will see how advances in therapeutic endoscopy can help us take care of various complications associated with IBD. We learn how techniques like endoscopic balloon dilatation, endoscopic stricturotomy, and enteral stents can help our patients with strictures. With more widespread availability of endoscopic mucosal resection, endoscopic submucosal dissection management of dysplastic areas in patients with IBD is more feasible now.
