Home as a context for health-care delivery is becoming increasingly important in the ecosystem of health-care delivery. The COVID-19 pandemic, combined with evidence supporting the value of telehealth and home-based care, plus a growing at-risk aging population with high functional needs, conspires to make the home an ideal setting for occupational therapy practices. This is the case for delivery of acute, episodic care such as posthospitalization needs, rehabilitative functional care to address long-term needs, and caregiver support and disease education. This chapter examines the system of home care and the role of occupational therapists in that context. Considered are the payment mechanisms, possible scope of practice, conceptual frameworks guiding practice, the specific benefits of home care provision for both clients/patients and occupational therapists, as well as the challenges of working in that space. In the current system of home care, occupational therapists have a critical role. Yet, they are also bound by payment mechanisms and reimbursement regulations that can hamper the ability of occupational therapists to work to their full scope of practice. Nevertheless, as the home continues to become more prominent as the context for health-care delivery, occupational therapists will increasingly have a central role in this emerging ecosystem.
