Immunosuppression in pediatric transplantation has evolved hugely within the last 30 years. Initially there were essentially only three drugs: cyclosporine, azathioprine, and prednisone. Currently most programs use induction regimens, and tacrolimus has become the maintenance immunosuppressive of choice. We discuss the range of immunosuppressive agents that are available and more recent developments. We then describe how these are combined into effective regimens and discuss the practical approach to different clinical scenarios.
