Abstract
An elderly woman with unexplained weight loss was extensively evaluated to exclude malignancy. She had a heavy cigarette smoking history with radiographic cavitation in the upper lobes with nodularity. The diagnosis of two separate and distinct atypical Mycobacterium—Mycobacterium avium intracellulare (MAI) and M. kansasii—was confirmed with three sputum samples and bronchoscopy. Atypical Mycobacterium should be considered in the evaluation of cavitary lung disease in patients with high risk of malignancy and MTB.
