Laryngopharyngeal Reflux and Pulmonary Manifestations
The AFS Textbook of Foregut Disease, pp 49-55
2023
Abstract
Reflux of gastric contents into the upper aerodigestive tract can result in a variety of laryngopharyngeal symptoms such as hoarseness, cough, asthma, and recurrent pneumonia. These symptoms are atypical for gastroesophageal reflux disease (GERD), and establishing reflux as their cause in the absence of typical GERD symptoms is challenging. Existing diagnostic tests lack sufficient sensitivity and specificity to confirm the presence of laryngopharyngeal reflux (LPR). Medical therapy has limited therapeutic benefit, whereas surgery provides a comparatively better outcome especially in carefully selected patients with early disease. The best surgical results are achieved in patients with normal esophageal clearance and abnormal proximal esophageal or pharyngeal acid exposure with a correlation between reflux events and symptoms.
