Reproducibility of Emergency Medicine Standardized Patient Cases from Philadelphia to Barcelona

Abstract
Adaptation of cases to a foreign language may pose difficulties in reproducing studies across countries. The Educational Commission for Foreign Medical Graduates has developed a ten station standardized patient emergency medicine program used to assess residents entering graduate training. The purpose of this study is to assess reproducibility of the examination from Philadelphia, USA to Barcelona, Spain and to explore issues related to the adaptation of a standardized patient examination. The cases were translated into Castilian Spanish by physicians in Barcelona, and a video tape of the ten cases as used in a recent US test administration was viewed by the Spanish physicians. Several discussions followed the translations, with questions and discrepancies addressed. The cases were then retranslated by an independent translator back into English and compared to the original cases. A video-tape of trained Spanish standardized patients’ performance was reviewed by the American physicians, resulting in modification of training for some of the standardized patients. Direct validation by an American physician was done in selected cases.
Twenty residents and junior faculty in Barcelona and fifty-six first and third year residents in Philadelphia participated in the assessment of these cases. Reliability of the source examination was 0.85. Evidence for construct validity of the source examination was demonstrated by significant improvement of scores after three years of training, and correlation of scores with faculty evaluation after six months of training. Analysis of reliability for the Barcelona examination indicated reliability of 0.84. Issues of adapting an emergency medicine clinical skills examination are discussed.
