Management of patients on the waitlist for cardiac transplantation includes appropriate medical, device‐related, and surgical management for advanced heart failure. This necessitates the use of beta‐blockers, angiotensin‐converting enzyme inhibitors, angiotension receptor blockers and/or aldosterone antagonists, diuretics, other vasodilators as needed, intravenous inotropic agents, and implantable cardiac defibrillators, along with cardiac resynchronization therapy as indicated. Surgical interventions may include mechanical circulatory support with ventricular assist devices or artificial hearts as bridges to transplant. Patients should also be allocated to the appropriate priority status when listed. While waiting for transplant, patients should be carefully followed for clinical deterioration, which might necessitate alteration of both therapy and status on the waitlist, and clinical improvement, which could result in a downgrade in status or even delisting. Attention should also be paid to antibody sensitization, although therapeutic approaches for desensitization remain unsettled.
