Abstract
Not since Eliot Freidson’s epic treatise, Profession of Medicine [l], have we had a book not only jammed from cover to cover with provocative ideas but so full of empirical studies that it could almost function as a forty year state of the art review [2]. If you read this opening statement as an indication of how impressed we are with this monumental effort you are correct. [1st paragraph]
