Objectives
COVID-19 infections and deaths have disproportionately impacted African American and Latino communities in the United States, further compounding the health disparities affecting them. For inner-city youth, the death of a caregiver related to COVID-19 added yet another potentially traumatic event in addition to violent injury, community violence, child abuse, domestic violence, parental incarceration, or deportation. This presentation describes the use of trauma-focused CBT (TF-CBT), trauma-and grief-focused components supplemented by case management for youth who experienced COVID-19–related deaths and violence.
Methods
Arturo Zinny, MA, describes the core TF-CBT components summarized by the acronym PRACTICE (Psychoeducation and parenting, Relaxation methods, Affective expression and regulation skills, Cognitive coping skills and processing, Trauma narrative and processing, In vivo exposure, Conjoint therapy sessions, Enhancing personal safety and future growth), as well as the TF-CBT grief-focused components (grief psychoeducation, naming the loss, preserving positive memories, committing to ongoing relationships, and treatment closure), how these are supplemented with case management and peer services at a hospital and community-based violence intervention program in Philadelphia, and how they are delivered via a hybrid approach, telehealth and in person, consisting of weekly sessions with the children and their caregivers.
Results
A sample of 52 African American and Latino youth who received TF-CBT supplemented with case management and peer services experienced significant improvement in PTSD symptoms from pretreatment to posttreatment as measured by the Child PTSD Symptom Scale, from a mean of 33.75 to 18.14 (p < .05).
Conclusions
Inner-city minority children are exposed to multiple traumas and chronic community violence that are often layered onto the traumatic death of a caregiver or other important person. Untreated PTSD can have devastating consequences for children, their families, and communities. The integration of TF-CBT with peer support and case management is an innovative approach to improving outcomes for underserved racial and ethnic minorities exposed to ongoing trauma and the COVID-19–related death of a caregiver.
