Microbicidal treatment of HIV-1-infected breast milk as an alternative for prevention of mother-to-child transmission of HIV-1 through breastfeeding

Abstract
Prevention of transmission of HIV-1 through breast milk would help reduce the number of pediatric cases of HIV/AIDS. Sodium dodecyl sulfate (SDS) is a broad spectrum alkyl sulfate microbicide that inactivates HIV-1 in vitro at low concentrations(2), and is inexpensive. Thus, alkyl sulfates may be used for microbicidal treatment of expressed HIV-1 breast milk. Within minutes of treatment, SDS (greater than or equal to 0.01%) inactivates in vitro cell-free and cell-associated virus added to breast milk. If SDS is removed after treatment, HIV-1 infectivity is not recovered. Flow cytometry revealed loss of cells upon treatment with 1% SIDS. At less than or equal to 1%, SDS decreased viral load in milk samples to undetectable levels. Alternative microbicides are currently being tested.
