The reasons for the lack of acceptance of an advanced ehealth system by carers at seven care organizations in the United States, the United Kingdom and the Netherlands are explored. Given that the design of the electronic record system was largely driven by the carers who used it in the seven organizations and that the system worked, the lack of acceptance was initially perplexing. Four reasons are put forward to explain this lack of acceptance: 1) the ability of the system to be used to evaluate job performance; 2) the pilot study model; 3) reluctance on the part of the carers to alter work routine; and 4) the system's introduction is premature.
