Medication management in individuals with persistent schizophrenia

Abstract
Participants for this study consisted of 41 male schizophrenics between the ages of 18 and 65 who were Forensic or Ex-Forensic residents living in an inpatient psychiatric facility. Each participant completed a battery of neuropsychological tests, a questionnaire regarding knowledge of, and attitude towards medication, and a practical, experimental measure, The Cognitive Screening for Medication Self-Management (CSMS). Each participant was also rated on an item from the Structured Clinical Interview for the Positive and Negative Syndrome Scale (SCI-PANSS) which relates to lack of insight and judgment. The purpose of this study was to determine if individuals with persistent schizophrenia demonstrated cognitive difficulties directly related to medication management. A second goal of this study was to identify which variables, from the assessment instruments, proved to be the best predictors of level of success in a 3 Phase medication program. It was hypothesized, and the results of this study substantiated, that individuals with persistent schizophrenia demonstrate difficulties with frontal lobe/executive functions such as novel problem solving, semantic clustering, mental flexibility, organizational and planning abilities as well as with acquisition and recall of information and prospective memory abilities. Regression analyses produced five variables that were able to account for 62.2% of the variance in terms of level of success in the Medication Education and Management Program. Implications for the study are discussed in terms of potentially using a practical dose planning task from the CSMS as a screening device to detect which individuals are likely to be successful with medication management.
