Early detection of decompensated heart failure: implementation of a process for remote heart failure monitoring utilizing implantable cardiac devices

Abstract
Heart failure (HF) is the most common cause of hospitalization in elderly patients in the United States and carries a high rate of morbidity, mortality, and health care utilization. Many HF patients receive an implantable cardiac device as part of current guideline directed medical therapy. Implantable cardiac devices are now monitored remotely, and many have the capability of detecting early warning signs of HF exacerbation. Monitoring these patients remotely for signs of HF has been found to decrease hospitalizations, morbidity, and mortality. An evidence-based approach to effectively utilize remote HF monitoring data was developed based on a comprehensive review of the available literature. A quality improvement project was designed to identify patients with these devices and implement a process for remote HF monitoring. The goal of this project was to create a process to obtain and review monthly and alert transmissions, use a multidisciplinary team approach to contact high risk patients and facilitate appropriate management by their heart failure or cardiology provider. This project was implemented over an 8-week pilot period and measured the percentage of alerts that were successfully acted upon utilizing this process. During the pilot there were a total of 78 alerts and 72 of the alerts were handled according to the protocol for an overall adherence of 92%. Potential revenue and total nursing and advanced practice provider time spent managing alerts was tracked to determine sustainability of the project. This exciting technology has the potential to positively impact hundreds of patients.
