Implementation and evaluation of a multi-step opioid monitoring program

Abstract
The opioid crisis resulted in several opioid prescribing guidelines that directly affect the treatment of chronic pain. Though designed to reduce opioid dependence, addiction, morbidity and mortality, opioid prescribing guidelines are largely ineffective as health care provider adherence to guidelines remains less than 50%. Providers criticize the guidelines for their impracticality and applicability, believing them to be a threat to their professional autonomy and patient-provider relationships. Opioid therapy remains a critical piece to treating chronic noncancerous pain and can be safe and efficacious, even at high doses, with appropriate monitoring. The purpose of this quality improvement project was to implement and evaluate an evidence based, multi-step opioid monitoring program to increase guideline-concordant care and ultimately improve patient, provider, and practice safety. The project utilized the Plan-Do-Study-Act (PDSA) methodology as the guiding framework to develop, implement, and evaluate the program. Across 4 providers, there was a total of 486 encounters during the studied period. Over the measured eight weeks, aggregate adherence to each of the five program steps was significantly greater than the goal of greater than 50%. Safer clinical environments improve patient outcomes, and integration of this program into standard pain management practice could improve patient and provider experiences. The fundamentals established in this program could potentially serve as an organizational foundation for other pain management practices looking to improve patient and provider safety.
