Implementation of a postoperative anesthesia handoff communication checklist to reduce omission errors

Abstract
Background: Effective communication during patient handoff between anesthesia personnel and the postoperative anesthesia care unit (PACU) is a crucial element of safe patient care. A breakdown in communication can result in the loss of important information, which can have a negative impact on patient care and compromise safety. To facilitate effective communication during handoff, utilization of a handoff checklist can help ensure consistency in providing essential information while minimizing omissions or errors. Methods: To improve the postoperative handoff practice within the local context, a quality improvement methodology was employed to reduce omission errors during postoperative patient handoff between anesthesia providers and post-anesthesia care unit (PACU) nurses. Compliance with the proposed handoff checklist was assessed, and the key elements identified were evaluated for the potential loss of information during handoff. The methodology utilized was the Plan-Do-Study-Act (PDSA) cycle to develop, implement, and assess the impact of a handoff checklist to reduce omission errors. Education sessions and engagement efforts were employed to introduce the proposed QI initiative with the aim of reducing communication deficiencies and increasing compliance with the proposed evidence-based postoperative handoff practice.
