Increasing nursing's knowledge of CHG treatment: a quality improvement project

Abstract
Central line-associated bloodstream infections (CLABSIs) are hospital-acquired infections that can increase patient mortality rates and costs for healthcare organizations. A DNP quality improvement (QI) project on a medical-surgical unit in an urban community hospital was implemented to improve chlorhexidine gluconate (CHG) treatment and documentation using AHRQ guidelines. CHG treatment has been shown in the literature to reduce CLABSIs when used appropriately by nurses. The QI project aimed to reinforce the evidence-based practices nurses learned in their onboarding and orientation classes through materials from AHRQ guidelines and an education intervention. The QI project utilized the plan-do-study-act (PDSA) cycle. A pre- and post-review of charts determined the number of times CHG treatment and documentation was performed using a data collection tool. Pre- and post-intervention analysis compared the number of opportunities to perform CHG treatment and documentation based on the number of central lines on the medical-surgical unit. The Chi-square test was used to analyze the pre- and post-documentation results. The results were not statistically significant (p = 0.12). Post-intervention, CHG treatment and documentation increased from 75% to 90%. There was also an increase in the number of times patient education related to CHG treatment and patient documentation of CHG refusal were documented. The education intervention reinforced the nurses' knowledge of CHG treatment and documentation. Education reinforcement provides the support nurses need to follow evidence-based best practice guidelines to prevent CLABSIs in the non-ICU setting.
