Abstract

Pancreatic ductal adenocarcinoma (PDAC) remains one of the leading causes of cancer related morbidity and mortality, with limited survival despite advances in cancer treatment. As life expectancy increases, octogenarians represent a growing and clinically important subgroup of patients with PDAC. Decision-making in this age group is particularly challenging owing to comorbid conditions, limited physiologic reserve, and quality of life considerations. While pancreaticoduodenectomy (PD) remains the only potentially curative treatment for localized PDAC, definitive radiotherapy (dXRT) has emerged as a viable alternative for select patients. However, national-level comparisons of survival outcomes between PD and dXRT, specifically among octogenarians with nonmetastatic PDAC, remain limited, providing little guidance to clinicians managing this vulnerable population.
