Abstract
An interview questionnaire evaluation checklist tool that includes all fundamentals of a best-practice interview questionnaire including the mode of questionnaire, questionnaire design, maximization of response rates, bias reduction, types of questions, question content, question wording, question order, question format and recall aids. The checklist tool was used to evaluate three Environmental Protection Agency (EPA) community interview questionnaires that were conducted at Superfund Sites and an Army Corps of Engineers Former Nansemond Ordnance Depot questionnaire. The evaluation checklist tool provides a quantitative evaluation grade on the entire questionnaire, each individual question and the collective questions. The evaluation checklist also provides a qualitative evaluation for each individual question.
