Abstract

Transition Services Transplant Patients Gift of Life Donor Program Continuous Quality Improvement Public Health
Transition programs play a critical role in ensuring stable health and quality of life for transplant patients transitioning from pediatric to adult care. This project is a quality improvement project that seeks to identify what types of transition services are available in hospitals within the Gift of Life Donor Program network to ensure a smooth transition from pediatric to adult care. The methods include a thorough literature review on the developmental stages of pediatric patients and the best practices of transitional services from consensus statements for transition programs. Surveys were sent out to social workers to collect information on the current transition services offered in hospitals within the Gift of Life Donor Program Network. Six criteria were established based on the best practices to evaluate the services of the hospitals in the network. A five-star rating system was established to assess the capacity of hospitals in fulfilling each of the six criteria. The results of the survey indicated that hospitals in the network have strong communication but weak planning in transition programs. Transition services in these hospitals have strong culture but poor policy. Short-term recommendations for organizational improvement included conducting an assessment of the patient's developmental stages, beginning the transition process early, and creating a formal transition policy or detail plan. Long-term recommendations included creating a formal transition team and evaluating the transition services. Elements of Continuous Quality Improvement were applied to the assessment results and recommendations for quality improvement of transition services.
