Abstract

Team-based care Patient Centered Medical Home
The Aim of this capstone is to assess the expected practice of team-based care within a Patient Centered Medical Home (PCMH) compared to the current practice of a microsystem in a community care setting and provide a recommended action plan by May 2024. The microsystem population for this capstone is set in a Federally Qualified Health Center (FQHC) in a rural area of Southeastern Massachusetts. The clinical microsystem represents a small group of professionals who work together to provide care to a defined patient population. The Microsystem is 1 of 3 health centers within this specific FQHC organization. The clinic was certified as a Patient Centered medical home 2018, but since Covid the clinic's operations have veered from previously established workflows. This capstone utilizes a gap analysis to compare the current state of operations and outcomes with the expectations of team-based care in the Patient Centered Medical Home. The results of this analysis show that the microsystem has room for improvement in operational measures specifically regarding daily huddles, care coordination meetings and site level meetings. Implementing team communication opportunities in the clinic schedule could improve performance on quality measures for all clinician teams. Although there are operational costs to providing protected time for team communication these efforts could increase reimbursement for the clinician teams and improve patient outcomes in the microsystem Keywords: Team-based care, Patient Centered Medical home
