Abstract
Methodology: From January 2014 to August 2016, this observational time series study utilized multiple planned sequential interventions to test pediatric patient centered transition (PACT) bundles in 3 different medical centers in 2-17 year old patients with asthma. Results: Data from all sites (n = 700) showed both, an improvement in composite pre-hospital discharge rates to 54% (process), and parent/caregiver teach back of essential home management skills during follow-up phone call to 73.33% (outcome).
