Abstract
Changes in the health care delivery system present a multitude of challenges to the home care clinician. The level of acuity is much higher and the use of technology is on an upward trend. Wound and ostomy care, with thousands of products, multiple wound etiologies, and limited reimbursement present frustration for client and caregiver. In this environment, planning for effective care is essential. This article reviews the physiology of wound healing and identifies goals for the promotion of wound healing. Product selection is correlated with wound assessment and the goals of care. Wound care is discussed in light of available technology, along with the principles for ostomy and fistula management.
