Hospital Readmission in Left Ventricular Assist Device (LVAD) Patients Compared to Patients Receiving Intravenous Inotropic Therapy

Abstract

Patients with advanced heart failure (HF) have improved survival with LVADs. Some patients receive intravenous, continuous milrinone as support prior to transplant. Hospital readmission are frequent events in patients with HF. There is little data on hospital readmissions with LVAD compared to milrinone. The aim of our study was to compare the readmission and HF exacerbation rates for milrinone patients compared to LVAD patients. Readmission rates were compared for patients receiving milrinone to patients with LVADs at a single-center. The comparison studied readmission and HF exacerbation rates between 2007 and 2011 at 1,3,6 and 9 month and 1 year intervals. A total of 64 patients were evaluated. Hospital readmission rates for LVAD and milrinone patients were compared for both all-cause diagnoses and specifically for HF. The mean number of all-cause hospitalizations for LVAD patients was 3.22 compared to 5.70 for milrinone patients at one year (p=0.0074). The mean number of HF exacerbations for LVAD patients was 0.84 compared to 2.20 for milrinone patients at one year (p= 0.0039). These findings were also observed at 1, 3, 6 and 9 month intervals.
