Polymorphous adenocarcinoma of the breast: a case report and review of the literature

Abstract
Background: Polymorphous adenocarcinoma (PAC) of the breast is a rare tumor that is morphologically similar to PAC of the salivary glands. There are few reported cases of PAC of the breast, and thus little is known about its behavior and best treatment. The purpose of this case report is to describe a case of PAC of the breast that had a unique presentation. Case Description: We report a case of PAC of the breast in a 56-year-old woman which presented as a benign-appearing dermal nodule. Immunohistochemistry of the tumor in this patient was estrogen receptor (ER) negative, progesterone receptor (PR) negative, and human epidermal growth factor receptor 2 (HER2) negative. The patient initially underwent excision of the nodule in clinic due to its presumed benign nature; when pathology was confirmed, she underwent re-excision for wider margins, sentinel lymph node biopsy, and adjuvant radiation. She has remained recurrence-free to date. Conclusions: PAC of the breast is a rare tumor with only four other cases reported to date. Due to the rarity of these neoplasms, only limited data is available in the current literature regarding best management. To the best of our knowledge, this is the fifth reported case of PAC of the breast and had a unique presentation as a benign-appearing dermal nodule. Like 3 of the 4 previously reported cases, this PAC appears to have an indolent growth pattern and less aggressive behavior than typical triple-negative breast cancers.
