Severe abdominal pain and diarrhea in an elderly woman receiving chemotherapy for colon cancer

Abstract
Patient 72-year-old woman.
Chief Complaint The patient was completing the second cycle of her chemotherapy regimen for colon cancer when she developed worsening abdominal pain and diarrhea. She did not have fever, oral mucositis, or severe leukopenia. Her chemotherapy regimen consisted of once-a-week infusions of 5-fluorouracil (5-FU) and Leucovorin for 6 weeks of an 8 week cycle (Roswell Park regimen). Three days after she received her 16th treatment, her abdominal pain became so severe that she required urgent medical attention and hospital admission. She underwent an extensive work-up to identify the etiology of her pain and diarrhea. A colonoscopy showed diffuse mucositis of the small bowel.
Medical History She had a 20-year history of ulcerative colitis and adenocarcinoma of the sigmoid colon, stage IIIC, diagnosed 5 months prior to hospital admission.
Surgical History Hysterectomy, cholecystectomy, and a subtotal colectomy for colon cancer.
Social History She denied any cigarette or alcohol abuse.
Family History She had no family history for cancer or inflammatory bowel disease.
Physical Examination Diffuse abdominal tenderness without any palpable abdominal masses, hepatomegaly, or splenomegaly, Neurological exam was grossly non-focal.
