A therapeutic serum level of phenytoin (PHT) is difficult to achieve in patients who receive enteral tube feeding. In four patients, PHT capsules were dissolved in 250 ml warm water and cooled to body temperature before being administered. A therapeutic serum level of PHT was maintained in all four patients. Another group of three patients received PHT suspension, at an interval of 8 h, with water at room temperature. Again serum PHT levels were maintained in the therapeutic range in all three patients. The present preliminary study indicates that bioavailability of PHT is increased by dissolution in water in patients receiving chronic enteral tube feeding. There is a need for further investigation of the kinetics of absorption of PHT and other antiepileptic drugs in patients receiving enteral tube feeding.
