Abstract
This chapter provides a review of the historical developments in psychiatry and psychology research that led to a focus on individual treatments—medications or psychotherapy—rather than on a combination of the two. The weaknesses of typical efficacy trials in distinguishing what treatments might help patients in “the real world” and the potential mechanisms that could influence combined treatment are discussed.
